The type of personal information we collect

We currently collect and process the following information:

• Treatment consent form.
• We collect notes from our conversations with you, details of any complaints or comments you make.
• If you are requested to show a photo ID to proof you are over 18 years of age we will take a photo copy and attach it on the treatment consent form.

• When you contact us by any means with queries, complaints etc.
• When you choose to complete any surveys we send you
• When you comment on or review our products and services
